The memorial is a white marble tablet, inscribed with names of those who served in the Second World War from Koloona. The names are divided into three columns and further divided by service branch. These sub-sections are: Australian Imperial and Military Forces (A.I.F. and A.M.F.), Royal Australian Air Force (R.A.A.F.), Women's Auxiliary Air Force (W.A.A.F.), and Australian Women's Army Service (A.W.A.S.).

The memorial is located in the foyer of Warialda Soldiers' Memorial Hall
