The memorial is a black granite plaque, inscribed with the names of those from Dignam's Creek who served in the First World War. The names are highlighted in white and are out of alphabetical order. 

The roll was unveiled in Dignam's Creek Hall on 16 September 1919. At a later date, it is was attached to the Cobargo and District Soldiers Memorial.
