"On January 28, 1941 an Avro Anson Aircraft No A4-5 of No 1 AIP navigation school RAAF Parkes, crashed in this area whilst enroute from Parkes to Mascot on a medical evacuation flight. All five airmen on board perished in the crash," as transcribed from the plaque.

The plane when approaching Glenbrook in low cloud appeared to break up with a failure in the middle of the port wing which resulted in a heavy crash in Clifton Avenue on the corner of Lucasville Road Glenbook, NSW, at around 4.30 pm. One engine smashed a power pole in Lucasville Road causing heavy sparking. Body parts were left all over the area and were still being found weeks later.

The memorial was established in 1994 by the RAAF who held a dedication service on March 24, 1994. The memorial is a small concrete plinth with a sloping front section which contains a bronze plaque with the wording above. It is located on the corner of Clifton Avenue and Lucasville Road Glenbrook on council land next to the corner property fence. It is easily missed due to its small size.
