The memorial is a large brass roll of honour, surrounded by a simple timber frame. The names of those who served in the First World War are inscribed across seven plaques, positioned side-by-side, which make up the main body of the roll. Above these is a longer, rectangular plaque, which is inscribed with the dedication.

The roll is mounted under a stained glass window, on the western side of St Columba's Presbyterian Church in Guyra, NSW. It was made/designed by Castle and Son of Sydney, NSW.

The Guyra Argus, 21 October 1926, described preparations for the official opening of the church on Saturday, 13 November. The report described in detail the interior of the church and stated:

Immediately beneath the large central western window, a Memorial Panel or Honor Roll, containing the names of the members of the Presbyterian Church who made the supreme sacrifice or served in the A.I.F. in the Great War of 1914-'19, will be set up with an oak bordering.

This memorial is the gift of one who was involved with the life and work of the congregation during the war period and subsequently.

In their coverage of the opening, the Argus, 18 November 1926, again highlighted the presence of the roll:

Other internal features which have won favourable comment are the beautiful furniture in the east end, and the rich and worthy brass Honour Roll under the large western windows.

The roll remained an important feature of the church. During the Anzac Day service on Monday, 25 April 1927, it was "suitably draped, and on it were placed wreaths of laurel and rosemary" (The Guyra Argus, 28 April 1927).

Access to the memorial is restricted to the opening hours of the church.
