This simple bronze plaque is located in Captain Cook Park in Warialda, NSW, alongside the Warialda Cenotaph. It is mounted in a metal frame and positioned in a garden. 

The plaque is inscribed with a dedication to the fallen and the unknown Australian soldier. It also commemorates the 75th anniversary of Remembrance Day. 

In 1993, the Australian Government exhumed the remains of an Australian soldier from the Adelaide Cemetery near Villers-Bretonneux in France. The Unknown Soldier was ceremoniously entombed in the Hall of Memory at the Australian War Memorial in Canberra. The plaque was unveiled on 11 November 1993 in recognition of this event. 

Various commemorative services are held in at the cenotaph throughout the year.
