The Coolatai Anglican Church First World War Honour Roll is a carved timber board, made of maple. It has an overall rectangular shape, with a curved pediment at the top featuring a carved Rising Sun emblem with a crown underneath. It also has small supporting shelves at the top and bottom and carved columns on each side.

The memorial bears the names of Lance Sergeant George Albert Cawkwell and Lance Corporal Jesse Judge, who died in the war. It was unveiled at the Coolatai church in April 1922 and dedicated by the Reverend R.H. MacLean (Warialda Standard and Northern Districts' Advertiser, 24 April 1922).

The memorial is located in the Coolatai Anglican Church and can be viewed when it is open to the public for services. A photo of the memorial is also attached to the nearby Coolatai Founding Families War Memorial Monument.

George Albert Cawkwell of Coolatai enlisted as a single man, aged 22 years. He sailed with the 33rd Battalion from Sydney on 4 May 1916. As a Lance Sergeant he was killed at Messines, Belgium on 7 June 1917. He was buried in the Toronto Avenue Cemetery in Belgium.

Jesse Judge of Coolatai enlisted as a single man, aged 27 years, on 12 January 1916 in the 33rd Battalion. His unit embarked from Sydney on 4 September 1916. As a Private in the 9th Machine Gun Company he died of wounds on 2 April 1916 in France and was buried in St. Sever Cemetery Extension, Rouen, France.
