This metal plaque is dedicated to the memory of three members of the Cousens family, including veteran Frank Page Cousens. It is surrounded by a timber frame and is attached to a wall in St John's Anglican Church in Tamworth, NSW. 

Frank Page Cousens was born in the Parry County and the Tamworth Parish. A grazier, aged 32 years and 2 months, he enlisted in the Australian Light Horse (A.L.H.) on 12 March 1915. Prior to his enlistment, he was a commissioned officer in the A.L.H. in Tamworth.

Frank was in a training squadron for the A.L.H. Regiment when he died at the Sydney Hospital on 14 July 1915, after suffering a fall down a flight of stairs. His obituary was published in The Tamworth Daily Observer the following day. At the time of the accident, he was attached to the Light Horse Depot at Liverpool, NSW. He was the eldest son of Mrs Cousens of 'Prinsted', Tamworth, and the late H. L. Cousens. Following a service at St John's, he was buried on 15 July in the Church of England Cemetery at West Tamworth.
